Quick answer: it depends on where your driving licence was issued. Drivers from the EU, UK, and most European countries can legally drive in Albania on their national licence alone. Drivers from the US, Canada, Australia, and several other non-European countries need an International Driving Permit alongside their national licence.
Who can drive in Albania without an IDP
If your licence was issued in an EU or European Economic Area country, it's valid in Albania on its own. The same applies to licences from the UK, Switzerland, and a handful of other European states with recognition agreements in place. You don't need to carry anything beyond your normal licence and passport.
Who needs an IDP
Albania recognises the 1949 Geneva Convention on Road Traffic, not the 1968 Vienna Convention. In practice this means licence holders from the United States, Canada, Australia, and other countries outside that recognition list should carry an International Driving Permit alongside their national licence. Some rental agencies will hire a car to you without checking for one, but that doesn't mean you're covered if a traffic police officer asks for it, or if you're involved in an accident and your insurer wants to see valid documentation.

How to get an IDP before you arrive
An International Driving Permit has to be issued in your home country before departure. It can't be arranged once you land in Tirana. In most countries this means applying through your national automobile association (the AAA in the US, the CAA in Canada, or your local motoring body elsewhere), and it's usually a same-day process with a passport photo, your existing licence, and a small fee.
It's worth doing this a couple of weeks ahead of your trip, particularly during peak season when processing queues can back up.
What happens if you're stopped without one
Albanian traffic police do carry out routine checks, especially around Tirana and on the coastal road toward the Riviera. If you're required to hold an IDP and can't produce one, you risk a fine, and in some cases your travel insurance may not cover you if something goes wrong while you're driving without valid documentation. It's a small, avoidable risk for something that takes almost no effort to sort out before you leave home.
Do rental companies in Tirana check for one
This varies by agency. Some check thoroughly at pickup, others don't ask at all. Neither approach changes the legal requirement. Assume you need one if your licence falls outside the EU and European recognition list, regardless of what happens at the counter.
